I consent to ZTM processing my personal data in the form of a telephone numer or residence address (depending on the personal data provided) for the purpose of providing by a phone or a letter information related to the issuing and using of the personalized Warsaw City Card (WKM). I have the right to withdraw my consent at any time and withdrawal of consent does not affect the lawfulness of the processing that was carried out on the basis of consent before its withdrawal.

I consent to ZTM processing my personal data contained in this application for the purpose of issuing and enabling identification of the person requesting the issuance of the WKM, in the event of the another WKM being issued in case of its loss, in case of giving back a ticket encoded at the WKM and for the purpose of providing (in relations to the e-mail address) information related to the issue and use of the WKM. I have the right to withdraw my consent at any time, and withdrawal of consent does not affect the lawfulness of the processing that was carried out on the basis of consent before its withdrawal. Withdrawal of consent is associated with the inability to continue using WKM. To withdraw your consent, you can send an email to: iod@ztm.waw.pl.

I declare that the graphic signs / photos / other elements I sent for posting on WKM do not violate the law, social norms, religious feelings and the goods of third parties, as well as the rights of other users. In the event that the above statement turns out to be false, I am responsible for any violations of industrial property rights, copyrights, personal rights and other property and I declare that in the event of disputes and claims from third parties, I will satisfy all legitimate claims of such persons, thereby releasing the contractor ( ZTM) with all liability in court proceedings.
